MDS Nurse RN

This role is responsible for coordinating resident assessments, ensuring accurate documentation, and supporting quality care planning while maintaining regulatory compliance.
Requirements
- Active RN license in good standing
- MDS experience in skilled nursing or long-term care preferred
- Working knowledge of PDPM, QMs, Care Area Assessments, and CMS submission processes.
- Strong organizational and documentation skills
- Passion for delivering excellent resident care
Responsibilities
- Coordinate and complete MDS assessments in accordance with CMS requirements and facility policy
- Review Quality Measures (QMs), 5‑Star metrics, and facility dashboards; help drive performance improvement initiatives.
- Ensure timely and accurate MDS submissions
- Collaborate with the interdisciplinary team to develop and update care plans; collect and validate supporting documentation from Nursing, Rehab, Social Services, Dietary, and Activities
- Monitor documentation to support PDPM and quality measures
- Participate in care plan meetings with residents and families
- Assist with survey readiness and audits
- Educate clinical staff on charting best practices that support accurate MDS coding and QMs
