MDS Coordinator (RN)
About The Position
Oak Grove Healthcare is seeking an experienced Registered Nurse to coordinate the Resident Assessment Instrument (RAI) process. The MDS Coordinator oversees completion of MDS assessments, CAA's, and care plans in compliance with Federal and State regulations, and helps ensure accurate care planning and reimbursement. The role involves monitoring and documenting the management of Medicare and Managed Care residents in collaboration with the interdisciplinary team. The successful candidate will be detail-oriented, compassionate, committed to resident-centered care, and interested in improving resident outcomes and processes.
Requirements
- Active RN license in good standing.
- At least 6 months of experience as an MDS Coordinator.
- Strong attention to detail and a commitment to excellence.
- Compassionate and dedicated professional.
Nice To Haves
- Experience in skilled nursing or rehabilitation.
- RAC-CT or RNAC certification.
Responsibilities
- Coordinate and participate in completing the Resident Assessment Instrument, including MDS, CAA's, and care plans, in compliance with Federal and State regulations.
- Monitor and document the management of Medicare and Managed Care residents in collaboration with the interdisciplinary team.
- Ensure resident assessments are accurate to support optimal reimbursement and high-quality care.
