About The Position

The MDS Coordinator works in a skilled nursing facility in the Hialeah area, coordinating resident assessments and supporting quality outcomes and regulatory compliance. The role involves completing the Resident Assessment Instrument (RAI) process accurately and on time, collaborating with interdisciplinary teams on care plans, monitoring reimbursement documentation, and supporting quality improvement and survey readiness. The coordinator also educates staff on MDS processes and documentation standards.

Requirements

• Active Florida nursing license.
• Strong knowledge of RAI, Medicare, and CMS regulations.
• Experience with care planning and interdisciplinary team collaboration.
• Excellent communication, organization, and documentation skills.

Nice To Haves

• Previous MDS Coordinator experience in a skilled nursing facility is preferred.
• RAC-CT certification is a plus.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
• Collaborate with nursing, therapy, social services, and other departments to develop and update care plans.
• Monitor Medicare and reimbursement documentation requirements.
• Maintain compliance with CMS guidelines and facility policies.
• Participate in quality improvement initiatives and survey readiness activities.
• Educate staff on MDS processes and documentation standards.
