About The Position

We are seeking an experienced and highly organized Registered Nurse (RN) MDS Coordinator to oversee Medicare assessments, care planning, and reimbursement-related activities within our Long-Term Care facility. This is an excellent opportunity for an RN who understands the MDS process, regulatory requirements, and the importance of accurate clinical documentation. Under the direction of the Director of Nursing – Long-Term Care, the MDS Coordinator plays a key role in ensuring that resident assessments and care plans are accurate, timely, clinically appropriate, and compliant with New York State Department of Health regulations and applicable reimbursement requirements. The MDS Coordinator must demonstrate the knowledge and skills necessary to support the care of adult and geriatric residents, including an understanding of age-specific needs, growth and development across the lifespan, and the ability to assess and interpret clinical information to support individualized care. Occupational exposure to bloodborne pathogens is reasonably anticipated in this position.

Requirements

  • Graduate of an accredited Registered Professional Nursing program.
  • Current New York State Registered Professional Nurse (RN) license.
  • Experience with MDS and care planning.
  • Familiarity with documentation requirements related to reimbursement.
  • Knowledge of New York State Department of Health regulations and the survey process.
  • Demonstrated leadership abilities.
  • Strong organizational skills and the ability to manage multiple priorities and meet deadlines.
  • Excellent communication and interpersonal skills.
  • Ability to handle confidential information with discretion.
  • Professional, courteous, and compassionate approach when interacting with residents, families, staff, and the public.
  • Computer literacy.

Nice To Haves

  • Bachelor of Science in Nursing (BSN).
  • MDS certification.
  • PRI certification.
  • Experience with Microsoft Office products.
  • Experience using MDS and/or care-planning software.
  • BLS certification through RQI upon hire.

Responsibilities

  • Coordinate and oversee the facility's MDS assessment process, ensuring assessments are completed accurately and within required timeframes.
  • Coordinate Medicare assessments, care planning, and reimbursement activities.
  • Develop, review, and update comprehensive resident care plans based on assessment findings and individual resident needs.
  • Ensure documentation supports appropriate reimbursement and reflects the resident's current clinical status.
  • Collaborate with nursing, rehabilitation, dietary, social work, medical providers, and other interdisciplinary team members to ensure accurate and comprehensive assessments and care plans.
  • Monitor compliance with New York State Department of Health regulations and regulatory requirements.
  • Participate in survey preparation and support the facility during regulatory surveys.
  • Identify documentation or care-planning concerns and work collaboratively with staff to address deficiencies.
  • Maintain confidentiality of resident and facility information and exercise sound professional judgment.
  • Meet established deadlines while effectively managing multiple priorities.
  • Promote teamwork, professionalism, dignity, respect, and exceptional customer service.
  • Serve as a positive role model and demonstrate effective leadership within the interdisciplinary team.
  • Utilize computer systems and applicable MDS/care-planning software to maintain accurate records.
