As a Medicare and Resident Assessment Coordinator(MDS), and the the Leader of the Interdisciplinary Care Plan Team, the incumbent will ensure that Minimum Data Sets (MDS), Resident Assessment Protocols (RAC), and Care Plans are completed in compliance with all applicable codes and regulations and in a timely manner and accurately address the holistic and individual needs of the resident. Conduct Care Plan meetings while interacting effectively with residents' family, physicians, therapists and other professionals, internally and externally. As an administrative nurse assumes leadership roles and responsibilities, identifies, develops and implements programing and services to enhance overall quality and ascertain positive outcomes.

Requirements

  • Must be a Registered, Licensed Nurse in good standing with the Texas Board of Nurse Examiners and have experience with Texas Long-Term Care Assessments and Care Planning.
  • High school diploma or equivalent required.
  • At least 6 months experience with Texas Long-Term Care Assessments and Care Planning.

Nice To Haves

  • Prefer Bachelor of Science in Nursing degree or higher.
  • At least six months’ experience with PDPM and MDS tools in long-term care.

Responsibilities

  • Works closely with the Director of Nursing (DON) and other administrative staff to ensure appropriate placement of residents in relation to current facility skill sets.
  • Ensures accuracy and coordinates with nursing staff on the level of care assignments for all residents; first upon admission, then with Care Plans as needed.
  • Responsible for accurate completion of all MDSs and Care Plans, ensuring they reflect the residents current status, as scheduled per Texas Regulations for Long-Term Care in accordance with Omnibus Budget Reconciliation Act, Medicare A, and other payer source reimbursement guidelines.
  • Communicates effectively with the Interdisciplinary Team (IDT) to discuss skilled residents progress or decline and overall medical status to ensure the best possible and most accurate RUG score can be obtained.
  • Tracks MDS schedule for residents and informs all disciplines of the selected Assessment Reference Date and completion date for the assessments.
