About The Position

Friendship Haven is seeking an experienced MDS/PPS COORDINATOR to join their team. This role requires an LVN or RN with extensive knowledge of Long-Term Care Medicaid, MDS, and Point Click Care. The coordinator will be responsible for overseeing resident assessments, ensuring regulatory compliance, monitoring documentation, and utilizing MDS software. They will also lead interdisciplinary meetings, update care plans, provide quality reporting, submit MDS data, and stay current with industry standards. Additionally, the role involves overseeing ancillary data collection and reporting.

Requirements

LVN or RN
MUST HAVE MDS/PPS EXPERIENCE
MUST HAVE PSSR EXPERIENCE
Extensive knowledge with Long-Term Care Medicaid, MDS and Point Click Care necessary
Must have 2 years of long term care experience
Must have MDS / TILE experience
Must be fluent in MDS 3.0
Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.

Responsibilities

Coordinate admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.
Follow all Federal and State guidelines for comprehensive assessments (MDS).
Review MDS and care plan files for accuracy, thoroughness, and organization.
Maintain expertise in software systems to ensure correct data entry and reporting.
Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
Adjust and revise care plans as needed based on current assessments and resident status.
Generate and distribute Quality Indicators Reports to leadership to support care improvement.
Ensure timely and accurate transmission of MDS assessments per reporting standards.
Attend professional development sessions to keep up with industry standards and best practices.
Collect and report ancillary services to corporate billing for accurate processing.
