About The Position
Beaumont Health Care Center is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our leadership team. This is an excellent opportunity for a skilled nursing professional who understands the critical role accurate MDS assessments, care planning, and reimbursement play in delivering quality resident care and supporting facility success. The ideal candidate will have experience with the MDS process, Medicare and Medicaid reimbursement, care plan development, and interdisciplinary team collaboration within a Skilled Nursing Facility.
Requirements
- Current Texas RN license in good standing
- Skilled Nursing Facility (SNF) experience
- Knowledge of MDS, RAI process, and care planning
- Strong organizational and time management skills
- Excellent communication and interdisciplinary collaboration abilities
Nice To Haves
- Previous MDS Coordinator experience
- RAC-CT certification strongly preferred
- Experience with PDPM reimbursement
- PointClickCare experience
- Knowledge of Medicare, Medicaid, and CMS regulations
Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure timely and accurate completion of all MDS submissions
- Review resident records to support appropriate clinical reimbursement
- Monitor assessment schedules and regulatory deadlines
- Maintain compliance with PDPM requirements and reimbursement guidelines
- Participate in interdisciplinary care plan meetings
- Assist in developing and updating resident-centered care plans
- Collaborate with nursing, therapy, dietary, social services, and medical providers
- Ensure care plans accurately reflect resident needs and services provided
- Conduct chart reviews and audits to ensure documentation supports coding accuracy
- Educate nursing staff regarding documentation requirements
- Monitor quality measures and identify opportunities for improvement
- Assist with survey readiness and regulatory compliance initiatives
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
- Support efforts to improve resident outcomes and facility performance metrics
- Track and analyze quality measures and reimbursement trends
