About The Position

Colonial Pines Healthcare Center is seeking an MDS Coordinator (LVN) to support its rehabilitative care facility in San Augustine, Texas. The role focuses on Medicaid and Medicare reimbursement and coordinates the completion of clinical assessments, supporting documentation, care planning, and transmission of MDS 3.0 in accordance with federal and state regulations. The coordinator also participates in and directs patient care according to facility clinical systems, regulatory requirements, and physician orders, while helping ensure high-quality, compassionate care in a customer-service-friendly environment.

Requirements

Current LVN license in the state of Texas.
At least 2 years of experience in a skilled nursing facility, including previous experience completing MDS assessments and care plans as an MDS Coordinator.
Demonstrated proficiency in RAI/PPS processes.
Strong attention to detail, documentation, and resident assessment skills.
Good communication, clerical, organizational, and computer skills.

Responsibilities

Maintain the MDS and care plan schedule for the interdisciplinary team.
Coordinate preparation of MDS assessments and care plans in accordance with regulatory requirements.
Track Medicare resident benefit days, validate the daily Medicare census, and coordinate this information with management to support accurate billing.
Complete responses to intermediary requests for additional information and appeals decisions within established time frames, as necessary.
Ensure Medicare denial letters are prepared for each resident discharged from Medicare coverage within Medicare-imposed time limits.
Participate in the ongoing evaluation of documentation required for Medicaid reimbursement.
Coordinate clinical assessments, supporting documentation, care planning, and transmission of MDS 3.0.
Participate in and direct patient care in accordance with facility clinical systems, regulatory requirements, and physician orders.
Help ensure high-quality services and a positive, compassionate patient experience.
