About The Position

Nadia Care is dedicated to improving maternal and infant health outcomes, particularly for underserved communities. We aim to transform maternity care by providing compassionate, trust-based engagement and comprehensive wrap-around support to expectant and new mothers and their families. Our services include care navigation, doula support, assistance with lactation, nutrition, housing, and transportation, leveraging technology for virtual engagement. We complement, rather than replace, primary obstetric care by addressing health-related social needs. Our core values are Empathy, Collaboration, and Resilience. We are seeking a full-time Registered Nurse (RN) to act as a clinical partner, bridging clinical care and community support for new mothers and babies. This role involves assessing medical needs, addressing postpartum concerns, and collaborating with Maternity Navigators to connect families with non-clinical resources. The position will help shape our care model in Washington, D.C., Montgomery County, and PG County, reporting to the Senior Manager of Care Delivery.

Requirements

  • Active Registered Nurse (RN) license in Washington, D.C. and Maryland compact state license
  • Based in the Washington, D.C. area, with the ability to conduct in-person visits in Southwest and Southeast D.C.
  • Minimum of 5 years of experience providing pregnancy and postpartum care in an outpatient pediatric, OB/GYN, maternal-fetal medicine, or similar ambulatory care setting
  • Exceptional listening and communication skills, with a strong ability to deliver compassionate, empathetic, and culturally-informed care
  • Proven problem-solver with excellent organizational skills and the ability to thrive in a flexible, fast-paced environment
  • Comfort and experience building relationships with patients virtually
  • Tech-savvy, with proficiency (or a willingness to quickly learn) platforms such as AthenaOne, Welkin, and MacOS for documentation and communication

Responsibilities

  • Conduct in-home assessments for moms and infants within 48 hours of the infant’s discharge from the NICU
  • Conduct telemedicine visits at set points in the postpartum period to identify unmanaged clinical needs and educate members on more complex clinical topics
  • Assess members that report Urgent Maternal Warning Signs or high blood pressure values, and help them navigate to the most appropriate care setting
  • Review blood pressure and Edinburgh Postnatal Depression Scale (EPDS) values and follow-up on irregular values that have not already been escalated
  • Serve as a clinical liaison between Nadia Care and the member’s obstetrics provider or primary care physician, documenting visit summaries and escalating all unmanaged clinical needs reported to the Care team
  • Help train Maternity Navigators (MNs) on prenatal and postpartum topics to ensure every member receives the same high-quality, evidence-based education
  • Be a voice for the community, bringing insights from your experience working with mothers and families in D.C. to help shape Nadia Care programs that truly meet their needs
  • Foster proactive collaboration between Maternity Navigators and clinicians, including case discussions, knowledge sharing on health education and community resources as needed during care team huddles.
