About The Position

This position supports the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program. The Community Health Worker works with high-risk pregnant and postpartum individuals to reduce infant mortality, maternal mortality, low birth weight, and prematurity. Responsibilities include community outreach, client education, screenings, resource navigation, referrals, and follow-up services to address barriers to healthy pregnancy and birth outcomes. The position collaborates with community partners, supports program data collection and documentation requirements, and participates in public health emergency response activities as assigned (Activation Level 1).

Requirements

High school diploma or equivalent.
One (1) year in Customer Service Setting.
One (1) year of Community Health Worker, or Navigator experience.
One (1) year of experience using Microsoft Word and Excel.
Valid US driver’s license without restrictions.
Current proof of automobile insurance.
Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment.

Nice To Haves

Two (2) years CHW experience, with a minimum of one (1) year as certified CHW.
One (1) year health education or maternal and child health experience.
One (1) year of data entry experience.
Associate’s degree in business or health-related field.
Completion of Community Health Worker training (Certified CHW).

Responsibilities

Prepare for and participate in community outreach events throughout Sedgwick County.
Build relationships with community organizations and referral partners to enhance service coordination.
Conduct outreach activities to identify and engage high-risk pregnant and postpartum individuals.
Manage and distribute approved client incentives.
Represent the Healthy Babies Program at community events while conducting MCH grant deliverables.
Conduct screenings and assess client needs to identify barriers to healthy pregnancies and birth outcomes.
Provide culturally appropriate health education on maternal and child health topics, including safe sleep, substance use cessation, mental health, and other identified needs.
Connect clients with community resources and services through referrals.
Follow up with clients to ensure successful connection to services and address additional needs.
Assist in evaluating the effectiveness of referral and support services.
Complete required screenings, assessments, and documentation according to program guidelines and grant requirements.
Maintain accurate, timely records and enter documentation within established timeframes.
Assist with data collection, surveys, needs assessments, and reporting activities.
Follow departmental policies, procedures, and workflows.
Identify opportunities for process improvement and recommend workflow enhancements.
