About The Position

Home & Health Care Management is seeking Master prepared Social Workers to join a Care Management Team (CMT). The CMT consists of a Master’s prepared Social Worker and a Registered Nurse, and together they provide comprehensive case management services to a target population. This role is ideal for individuals passionate about making a difference, detail-oriented, computer literate, and eager to work as part of a team supervising the care of clients receiving home and community-based services. The position requires strong problem-solving skills, knowledge of community resources, and a desire to help clients address social determinants of health care. The company values the significant contributions Social Workers make to client health and welfare. The work schedule is weekdays from 8 am to 5 pm, with no weekend work required.

Requirements

Master's degree in Social Work or another related field
Skilled in operating a personal computer utilizing a variety of standard software
Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
Your own vehicle and proof of current auto insurance
CPR certificate required within 30 days of hire

Responsibilities

Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant’s needs.
Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community.
Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination.
Provides education to participants so they can better understand the benefits of services and community supports.
Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.).
Explains eligibility requirements, coordinates transportation for participants and other basic needs of support.
Document all consumer interaction and maintain timely reporting and documentation requirements.
Observes participants behavior; reports significant changes to supervisor or team; documents appropriately.
Daily case conference with CMT about shared participants.
Performs other related duties as assigned.
