About The Position

Home & Health Care Management is seeking Master prepared Social Workers to join their Care Management Team (CMT). The CMT consists of a Master's prepared Social Worker and a Registered Nurse who will provide comprehensive case management services to a target population. This role involves working with older adults, children, and individuals with disabilities, utilizing various health programs to find solutions for clients. The ideal candidate is passionate about making a difference, detail-oriented, computer literate, and enjoys problem-solving and helping clients overcome social determinants of health care problems. The company values the contributions of Social Workers to client health and welfare and offers a weekday work schedule.

Requirements

  • Master's degree in Social Work or another related field
  • Skilled in operating a personal computer utilizing a variety of standard software
  • Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • CPR certificate required within 30 days of hire

Responsibilities

  • Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant’s needs.
  • Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
  • Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
  • Provides education to participants so they can better understand the benefits of services and community supports.
  • Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of support
  • Document all consumer interaction and maintain timely reporting and documentation requirements
  • Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
  • Daily case conference with CMT about shared participants
  • Performs other related duties as assigned.
