About The Position

PurposeCare is seeking a dependable Marketing Specialist, also described in the posting as a Care Transition Coordinator, to support client referrals and company growth. PurposeCare provides home care and home health services in the Midwest, coordinating home-based care for more than 3,000 clients each month.

The role develops PurposeCare’s Home Health and Home Care presence within supportive living and home- and community-based residences. It generates referrals through relationships with hospitals, healthcare systems, case managers, and other community resources. The position handles the complete referral process, from identifying home health needs and verifying benefits through coordinating care. It also represents the agency professionally, reports on marketing activity, and follows company policies, compliance requirements, and HIPPA regulations.

Requirements

• One year of home health, home care, marketing, or case management experience, or a combination of these.
• Valid driver's license.
• Excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet assigned deadlines.
• Sound judgment and good decision-making skills.
• Ability to organize and prioritize work.
• Ability to establish good working relationships internally and externally.
• Ability to sit at a desk and listen effectively for long periods on the telephone.
• Professional appearance and demeanor.
• Ability to use office equipment.
• Patient and friendly manner.
• Proficient computer skills.
• Ability to work evenings or weekends as required.

Responsibilities

• Participate in short- and long-range planning to achieve company goals and objectives.
• Manage the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
• Cultivate new and existing relationships to support company growth.
• Develop the Home Health and Home Care presence within supportive living and home- and community-based residences.
• Generate referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on marketing activities.
• Initiate and expand the referral network.
• Comply with company compliance requirements and HIPPA regulations.
• Perform other duties as assigned.
