Marketing / Admissions Director – Skilled Nursing Facility
About The Position
West Janisch Health Care Center is seeking an experienced, highly motivated Marketing / Admissions Director with a strong Skilled Nursing Facility (SNF) background, REQUIRED to lead census development, referral management, admissions coordination, and community outreach efforts for our facility. This is not an entry-level marketing role and not a desk position. We are looking for a knowledgeable SNF professional who understands hospital referral processes, managed care authorizations, rehabilitation admissions, census development, post-acute care transitions, and relationship-building within the healthcare community. The ideal candidate is a driven, relationship-focused professional who thrives in the field, builds trust quickly, and understands the urgency and follow-through required to successfully convert referrals into admissions.
Requirements
- Previous Skilled Nursing Facility (SNF) marketing, admissions, or census development experience REQUIRED
- Strong understanding of managed care authorizations, Medicare/rehab admissions, hospital discharge planning, and post-acute care operations
- Proven success in building referral relationships and driving census growth
- Strong communication, networking, and organizational skills
- Self-motivated, proactive, and highly responsive
- Ability to work independently while maintaining urgency and accountability

Responsibilities
- Develop and execute a strategic outreach plan to grow long-term care, skilled nursing, and rehab admissions
- Build and maintain strong referral relationships with hospitals, case managers, discharge planners, physicians, rehab providers, assisted living communities, and community organizations
- Conduct routine field visits, hospital rounds, and networking outreach
- Respond quickly and professionally to incoming referrals
- Follow up on pending referrals, authorizations, and discharge plans
- Coordinate admissions from referral to arrival, ensuring smooth transitions
- Work closely with clinical and operational teams to maximize census opportunities
- Track referral conversions and admissions trends
- Represent West Janisch Health Care Center within the healthcare market
- Promote the facility’s clinical strengths, outcomes, and services with confidence and professionalism
- Attend networking events, hospital meetings, and community outreach activities
