About The Position

Tembo Health is seeking a Market Engagement Manager to lead its Nevada operations. This role involves building relationships with primary care practices, specialists, and community partners across Nevada to ensure patients and caregivers facing dementia have access to Tembo Health's specialized care management and navigation services. The position requires strategic relationship building, data-driven practice management, and a hands-on approach to market operations and growth. It is a hybrid role with local travel required.

Requirements

5+ years in healthcare or managed care (Provider Relations, Population Health, Practice Management, or Customer Success).
Deep understanding of Value-Based Care and Medicare Advantage.
Experience with the challenges facing networks and primary care practices.
Thrive in a fast-paced startup environment.
Exceptional presentation skills, able to communicate with executives, physicians, and family caregivers.
Comfortable with EMRs (Athena experience is a plus), CRM tools, and data analysis.
Must be based in Nevada (Clark County preferred) with the ability to travel locally.

Responsibilities

Build and maintain long-term partnerships with key stakeholders, primary care practices, affiliate clinicians, and specialists.
Implement smooth EMR referral workflows to ensure Tembo Health is easily accessible for dementia support.
Support network and clinician efforts related to clinical performance to influence value-based care outcomes.
Present patient-level information and clinical insights to practices to identify and refer eligible families.
Monitor market-specific KPIs to ensure high clinician satisfaction and low referral friction.
Establish Tembo Health's presence with specialty clinics, community resource centers, home health agencies, and senior advocacy groups.
Execute local marketing initiatives to drive brand awareness and referral volume.
Own the Nevada rollout project plan and ensure milestones are met on time.
Act as a liaison between Nevada practices and internal teams to improve tools and workflows.
Provide monthly care navigation for a subset of patients, coordinating care and ensuring benefit utilization (bonus opportunity).
