Provides a leadership role in the evaluation, planning, implementation, development and maintenance of enterprise wide value based care operations for United Health Services and community based practices as well as other systems as identified. Works collaboratively with operations, population health and clinical leadership to develop these systems/service and ensure compliance. Serves as a resource for enterprise requirements for value based payment programs. Aides in the development of training, implementation, testing, and design within the electronic health record as it relates to data exchange with United Health Services partners. Identifies problems, determine needs, and develop recommendations for program compliance and financial success. Also, coordinates and supervises staff who analyze care gap data and outreach to patients. Participates in data analytics for performance improvement and lean six sigma projects.
