About The Position

The Manager - Pharmacy Clinical Outcomes will lead end-to-end pharmacy clinical program and systems initiatives. This role involves translating strategic priorities into actionable implementation plans, developing comprehensive business requirements documents, and serving as the primary liaison between Pharmacy and IT, including Epic. The position focuses on designing, implementing, and optimizing clinical decision support tools, defining and operationalizing patient cohorts, and supporting pharmacy registry development and population health reporting. The role also includes leading the implementation of pharmacy clinical programs, coordinating cross-functional efforts, facilitating governance structures, and defining/monitoring key performance indicators. Additionally, the manager will establish and assess internal controls and ensure compliance with University policies.

Requirements

  • Pharm D required
  • 340B certification required or completion within first 30 days of hire
  • Minimum of 5 years of relevant experience
  • Minimum of 5 years of pharmacy practice experience in a health system setting
  • Strong understanding of health system pharmacy

Responsibilities

  • Lead end-to-end pharmacy clinical program and systems initiatives by gathering requirements from clinical, operational, and financial stakeholders and translating strategic priorities into actionable implementation plans.
  • Develop and maintain comprehensive business requirements documents (BRDs), workflows, functional specifications, and project documentation to support pharmacy clinical, operational, and financial initiatives.
  • Serve as the primary liaison between Pharmacy and IT, including Epic, to ensure accurate translation of requirements into system design, build, testing, user acceptance, validation, and go-live readiness.
  • Lead the design, implementation, and optimization of clinical decision support tools, including Epic BPAs, by defining trigger logic, workflow integration, thresholds, and inclusion/exclusion criteria while balancing clinical effectiveness and alert fatigue mitigation.
  • Define and operationalize high-value patient cohorts, including specialty populations, high-risk patients, and adherence gaps, by partnering with analytics teams to develop cohort logic across EHR, claims, and pharmacy platforms.
  • Support pharmacy registry development, population health reporting, and targeted interventions that improve clinical outcomes, medication adherence, revenue optimization, and value-based care performance.
  • Lead implementation of pharmacy clinical programs, service line expansions, and workflow enhancements across specialty pharmacy, ambulatory services, transitions of care, and other pharmacy initiatives.
  • Coordinate cross-functional efforts across Pharmacy, IT, Nursing, Finance, Quality, and executive leadership to ensure alignment between strategic objectives and frontline execution.
  • Facilitate governance structures, working sessions, and executive reporting processes; proactively identify and escalate risks, trade-offs, and key decisions to support successful implementation.
  • Define, monitor, and report key clinical, operational, and financial performance indicators through dashboards and executive reporting to support data-driven decision-making and continuous improvement.
  • Establishes and continuously assesses the effectiveness of the internal controls within the unit and compliance with University policies and procedures. Ensures employees are trained on controls within the function and on University policy and procedures.
