About The Position

The Transitional Care Program (TCP) at UHN is a collaborative and innovative approach designed to help patients who no longer need acute care services, but often find themselves waiting for an alternate care facility, such as convalescent and long-term care. Patients and families have access to the specialized care needed in a restorative setting that allows for patient optimum functioning and quality of life. The TCP philosophy is all about delivering patient care in the most appropriate setting where patient privacy, dignity and safety are top priorities. Reporting to the Director of Professional Practice (DPP), the Manager of Professional Practice (MPP) for the TCP will be covering clinical programs at all 3 TCP sites: 82 Buttonwood Avenue, Toronto (Buttonwood RCC); 37 Austin Terrace, Toronto (Hillcrest Reactivation Centre), and 265 Queen’s Drive, North York (Queen’s Estate). The ideal candidate will have a nursing background, will be a member of the UHN Collaborative Academic Practice (CAP) portfolio, as well as a TCP Clinical Leader. The successful applicant will support the development, implementation and maintenance of profession-specific and interprofessional practice standards at the program level and across UHN, as required. The MPP will also be required to participate in the TCP on-call process, on a rotating basis.

Requirements

  • Current registration with the College of Nurses of Ontario and able to practice nursing without restrictions required.
  • Completion of Master’s Degree in Nursing, Education or Health Administration required.
  • Qualifies for clinical cross-appointment with the University of Toronto.
  • A minimum of 5 to 7 years related clinical experience.
  • A minimum of three years in clinical leadership role.
  • Experience in developing, implementing and maintaining professional practice standards and quality control measures in a health care environment required.
  • Consulting, influencing and negotiating skills.
  • Ability to think strategically and at an organizational and systems level, as well as at the program, site, unit and individual levels.
  • Broad knowledge of overall hospital structure, operating policies and decision making processes.
  • Knowledge of the Regulated Health Professions Act, standards of professional practice and codes of ethics, health care consent and ethical implications of care delivery.
  • Comfortable with use of technology; knowledge of computer systems and software applications.
  • Broad understanding of the health care system, related issues and trends, policies and legislation provincially, nationally and internationally.
  • Excellent knowledge of quality improvement principles and practices.
  • Sound analytical, critical-thinking, decision making skills and problem-solving skills.
  • Strong verbal and written communication and presentation skills.
  • Ability to collaborate effectively with diverse management and employee groups and individuals.
  • Ability to adapt, implement and manage changes and facilitate its acceptance by others.
  • Ability to work effectively and efficiently in new environments and culturally sensitive situations.
  • Ability to meet multiple and competing deadlines.
  • Ability to interpret, communicate and disseminate research findings to support patient care and recommends evidence-based policy and practice changes.

Responsibilities

  • Collaborates with other site DPPs, MPPs, other nursing practice and allied health leaders to ensure that practice standards are aligned and integrated, where possible, across programs, and organization wide.
  • Directly supervises a number of Advanced Practice Nurse Educators (APNEs) across the portfolio and maintains strong linkages with UHN’s practice-based education office.
  • Establishes, maintains and leads a number of quality improvement initiatives and works closely with the DPP as well as other practice and operational colleagues to promote a vision of practice excellence, integrating practice-based education and point-of-care research capacity within and across UHN programs, hospitals and in the external community.
  • Mentors diverse staff and learners.
  • Supports and partners with the site DPP to develop and implement care process changes, develop standards of work, inform organization-wide care delivery models, and deliver on practice-based education priorities.
  • Actively engages in organization-wide projects, most notably the optimization of UHN’s new health information system.
  • Participates in the TCP on-call process, on a rotating basis.
