Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community. The Housing and Health Integration Supervisor provides leadership and direct supervision across two complementary program areas: Heartland Alliance Health's Community Based Care services and a HUD-funded Chronic Homeless Supportive Housing program operated in partnership with the AIDS Foundation of Chicago (AFC). This is a full-time, single-site position operating five days a week. Across both programs, this role blends program oversight, contract compliance, health outcomes monitoring, and hands-on clinical behavioral health support in a low-barrier, trauma-informed environment. This role has a strong emphasis on care coordination and linkage to care for individuals experiencing homelessness, including connections to primary care, behavioral health, and psychiatric services. The supervisor provides direct supervision to the assigned Case Managers and Outreach Workers across both program areas.
