About The Position

Better Living Health and Community Services has a permanent full-time opening for the position of Manager, Hospice Palliative Care. This role will be responsible for the management of health, wellness, and continuity of service delivery for clients, residents, and their families. The Manager will also supervise Hospice Program staff and advance Palliative Care in the Better Living catchment area. This position will be split between Better Living Hospice program locations in Newmarket and Toronto.

Requirements

  • Undergraduate degree or post graduate education in a related health or social services discipline
  • Minimum of 5 years experience working in health or human services, or equivalent experience
  • Minimum of 3 years management experience leading a team in the delivery of health or human services
  • Working knowledge of psychosocial assessments, and working with electronic records and in a client data base system
  • Must possess a valid Ontario Driver’s License and access to a vehicle for the purposes of business travel

Responsibilities

  • Oversee the operations of the psychosocial team including case management, visiting hospice services, caregiver support, grief and bereavement and art therapy
  • Manage the day to day Human Resources within the department (including program students and volunteers), ensuring effective teamwork, high standards of work quality and organizational performance
  • Responsible for the growth, development, implementation and evaluation of results-based programs that improve and enhance client-centered service delivery and operational performance
  • Provide client and caregiver support in the absence of psychosocial staff at all hospice programs
  • Complete monthly statistical reporting, monthly and quarterly reports
  • Manage the Community Hospice budgets
  • Provide client and caregiver support in the absence of psychosocial staff
  • Participate in the rotational on-call system with Director of Care at Margaret Bahen Hospice
  • Support PalCare, and the NYTHP and NYSS OHT’s with education to health care providers and community members as requested
  • Pursue continuous learning and improvement by seeking out new ideas, identifying and achieving goals and using lessons learned to improve the performance and effectiveness of the hospice palliative care programs
  • Actively participate in the development of policies and procedures in compliance with accreditation standards, quality initiatives and funder requirements
  • Lead program promotion and community outreach activities
  • Support the writing of grants as applicable to hospice programs
  • Provide services in keeping with the organization’s Mission, Vision and Beliefs and Values.
  • Adhere to the principles and expectations outlined in the Code of Conduct.
  • Maintain compliance with the policies and procedures of the organizations.
  • Adhere to the Personal Health Information Protection Act (PHIPA 2010) as outlined in the organization privacy policies and procedures.
  • Perform the functions of job description in compliance with the Health and Safety practices of the organization and in accordance with the provincial Occupational Health and Safety Act.
  • Participate in the continuous quality improvement activities of the organization in order to advance a culture of quality and support ongoing compliance with CARF and HPCO Accreditation standards.
