We are searching for our next SUPER STAR! As the Director for Chronic Disease Prevention program is responsible for the initial start-up of the program with the goal to decrease the rates of morbidity or mortality due to chronic disease as outlined in the program plan. This position is responsible for hiring and training of program staff; and implementation of an evidence-based educational program for diabetes prevention The program is intended to serve 500 participants annually and conduct a minimum of 4,000 one-on-one nutritional sessions with a Registered Dietitian, and participant sessions with lifestyle coaches. The Director for Chronic Disease Prevention program is responsible for achieving program objectives related to the prevention of Type 2 Diabetes. The Director for Chronic Disease Prevention program is responsible for direct reports that includes Register Dietitian(s), Lifestyle Coaches and Customer Service Representatives. The Director of Chronic Disease Prevention will coordinate with the Vice President and Chief Medical Officer on the reporting on program participant’s progress to the applicable clinical team who will monitor progress for referred patients and provide additional encouragement and support to the patient.

Requirements

  • Bachelor of Science degree in Nutrition
  • Currently licensed as a Registered Dietitian (R.D.) in the state of Texas.
  • Five years project/program management experience
  • Three years supervisory experience.
  • Knowledge of personnel, administrative and financial procedures, and practices, as well as principles, techniques, terminology and procedures of sound management.
  • Judgment and decision making includes ability to apply general rules to specific problems to produce site specific solutions.
  • Must have ability to deal effectively with patients, providers, administration and the public.
  • Must have excellent skills in planning, organizing, and delegating.
  • Proficient Microsoft Office programs.
  • Demonstrated analytical and problem-solving skills.

Nice To Haves

  • Additional credential of Certified Diabetic Educator (CDE) preferred.
  • Experience in health education or health promotion program planning, management, and evaluation; community outreach; communications.
  • Experience in area of healthy communities or behavioral change interventions.
  • Demonstrated ability to work effectively in a collaborative environment.
  • Excellent communication skills, verbal and written plus strong organizational skills.
  • Spanish speaking preferred.

Responsibilities

  • Lead the implementation of prescribed evidence-based educational program for prevention of Type 2 Diabetes, to include staff training, development of materials, scheduling sessions and enrollment, engagement, and retention of referred individuals.
  • Responsible for development and implementation of metrics to monitor progress towards program goals.
  • Establish schedules for Registered Dietitian(s) to provide evidence-based educational programs.
  • Promotes patient access and appropriate utilization of program services.
  • Monitors performance of program through defined metrics.
  • Responsible for implementation and maintenance of schedules for education sessions.
  • Promote environment that supports 100% achievement of participant session goals.
  • Hiring, training, supervision, and development of staff to include work schedules, meetings & huddles, monitoring adherence to procedures & protocols.
  • Responsible for personnel activities to include interviewing and hiring of applicable staff, employee leave requests, payroll processing, preparation of Personnel Action Requests (PAR) for employment changes, addressing behavioral and performance deficiencies of staff.
  • Responsible for compliance with program expectations as established by CentroMed or as specified in grant with Bexar County or other outside entity.
  • Assists the Grants Management department with preparation of grant applications and submission of performance reports on adherence to program expectations.
  • Oversees achievement of specified measures specified for the Chronic Disease Prevention program and submits reports to applicable Compliance / Performance Improvement Program committees.
  • Assures that assigned staff comply with applicable laws, regulations or standards promulgated by federal, state, local or national accreditation agencies.
  • Actively promotes high levels of customer service within all areas of responsibility.
  • Understands, leads and promotes CentroMed’s mission and philosophy relating to ethics, integrity, safety, corporate responsibility and objectives.
  • Perform any other duties assigned that are in keeping with the mission, purpose, goals and objectives of the organization.
