About The Position

This position provides statewide training, technical assistance, and subject matter expertise supporting the Rural Health Transformation Program (RHTP), a federally funded initiative aimed at improving chronic disease outcomes in rural communities. The role serves within the CCCPH Branch and focuses on evidence-based diabetes and hypertension prevention and management, including guiding ROOTS Hubs in planning, implementing, and adapting interventions for diverse rural and underserved populations. Responsibilities include assessing Hub needs, supporting clinical-community linkages, strengthening provider engagement approaches, and advising on best practices in chronic disease program management. The position also coordinates data collection, monitoring, and analysis related to diabetes and hypertension outcomes to inform planning and evaluation.

Requirements

Bachelor’s degree from an appropriately accredited institution and three (3) years progressively responsible experience related to the area of assignment OR An equivalent combination of training and experience.

Demonstrated experience applying intermediate-level program management techniques to implement chronic disease interventions such as National DPP, DSMES, SMBP, and team-based care across diverse community or clinical settings.
Proven skill delivering structured technical assistance—including trainings, virtual office hours, and one-on-one consultations—to regional stakeholders implementing diabetes and hypertension initiatives.
Documented experience conducting risk-factor gap assessments and strengthening clinical-community linkages to support evidence-based cardiometabolic health strategies.
Applied proficiency in collecting, validating, and analyzing diabetes prevalence data, hypertension surveillance metrics, and program performance measures to support planning and evaluation.
Established track record implementing or supporting communication strategies and peer-sharing activities that promote adoption of diabetes prevention, glycemic control, and blood-pressure control interventions within rural or underserved regions.

Responsibilities

Assess regional ROOTS Hubs’ technical assistance needs related to diabetes prevention, diabetes self-management, hypertension prevention, and blood pressure control strategies.
Guide Hubs in selecting, adapting, and implementing evidence-based chronic disease interventions—including National DPP, DSMES, SMBP, and team-based care—to fit rural and medically underserved contexts.
Coordinate and deliver structured technical assistance through trainings, virtual office hours, one-on-one consultations, and peer networking sessions focused on diabetes and hypertension best practices.
Review Hub workplans, deliverables, and implementation progress, providing clear, actionable recommendations to strengthen fidelity and performance.
Support Hubs in conducting risk-factor gap assessments, developing clinical-community linkages, and integrating provider engagement strategies into chronic disease workflows.
Maintain and disseminate program materials, webpages, and guidance resources to ensure up-to-date content related to diabetes and hypertension interventions.
Facilitate Hub efforts to access and appropriately use diabetes prevalence and control data, hypertension surveillance data, and related evidence-based tools.
Coordinate data collection, validation, and analysis activities to support program monitoring, compliance, and reporting for diabetes and hypertension measures.
Compile metrics and outcomes for funder and stakeholder reports, including participation in diabetes prevention, SMBP implementation, and blood pressure control results.
Develop success stories highlighting improvements in diabetes and hypertension prevention and management achieved through Hub implementation.
