Responsibilities include assessing Hub needs, supporting clinical-community linkages, strengthening provider engagement approaches, and advising on best practices in chronic disease program management. The position also coordinates data collection, monitoring, and analysis related to diabetes and hypertension outcomes to inform planning and evaluation.
