This position provides technical assistance, guidance, and subject matter expertise to regional Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) Hubs implementing chronic disease goals within the Rural Health Transformation Program. The role supports program planning and execution across cancer prevention and control, early detection and screening, and tobacco/nicotine use prevention and cessation. Responsibilities include analyzing chronic disease trends, advising on evidence‑based clinical and community strategies, and strengthening provider engagement and multidisciplinary care models. The position also coordinates Hub data collection, monitoring, and interpretation related to cancer and tobacco outcomes.
