Chronic Disease Specialist
About The Position
This position provides technical assistance, guidance, and subject matter expertise to regional Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) Hubs implementing chronic disease goals within the Rural Health Transformation Program. The role supports program planning and execution across cancer prevention and control, early detection and screening, and tobacco/nicotine use prevention and cessation. Responsibilities include analyzing chronic disease trends, advising on evidence‑based clinical and community strategies, and strengthening provider engagement and multidisciplinary care models. The position also coordinates Hub data collection, monitoring, and interpretation related to cancer and tobacco outcomes.
Requirements
- Bachelor’s degree from an appropriately accredited institution and three (3) years progressively responsible experience related to the area of assignment OR An equivalent combination of training and experience.

- Demonstrated experience applying public and community health principles to chronic disease, cancer control, or tobacco/nicotine cessation initiatives, reflecting intermediate proficiency in program implementation.
- Proven skill delivering training and technical assistance to community or regional stakeholders, including facilitating structured consultations, virtual office hours, or capacity‑building sessions.
- Applied experience collecting, validating, and analyzing cancer‑ or tobacco‑related data to inform planning, performance monitoring, and evaluation activities.
- Demonstrated effectiveness communicating technical guidance to program stakeholders across diverse settings, including clinical partners, community organizations, and multidisciplinary teams.
- Documented experience leading or supporting initiatives aimed at reducing cancer burden or decreasing tobacco/nicotine use, including implementing evidence‑based interventions within rural or underserved populations.
Responsibilities
- Assess regional Hub needs related to cancer prevention, cancer screening improvement, tobacco/nicotine cessation integration, and chronic disease management.
- Guide Hubs in selecting and adapting evidence‑based chronic disease interventions for rural and medically underserved populations.
- Provide ongoing technical assistance through structured calls, trainings, office hours, and one‑on‑one consultations focused on cancer control and tobacco cessation strategies.
- Review Hub workplans, deliverables, and implementation progress to provide actionable recommendations aligned with program standards.
- Support Hubs in implementing screening quality‑improvement activities and provider engagement strategies within clinical workflows.
- Assist Hubs in integrating evidence‑based cessation services, including Quitline referrals, counseling approaches, and pharmacotherapy protocols.
- Coordinate the collection, validation, and analysis of cancer‑ and tobacco‑related data to support planning, monitoring, and evaluation.
- Compile data and performance metrics for required reports to funders and stakeholders, emphasizing screening uptake, cessation activity, and rural cancer burden.
- Maintain and disseminate cancer and tobacco‑related program resources, guidance documents, and tools to support Hub implementation.
- Facilitate peer networking and information‑sharing opportunities focused on effective chronic disease, cancer control, and tobacco cessation strategies.
