Requirements
- Bachelors of Nursing upon hire or 1-3 years experience equivalent required
- Minimum of three (3) years experience in Care Coordination or Case Management.
- Registered Nurse: CA (RN:CA) required
Nice To Haves
- Certified Case Manager (CCM) preferred
Responsibilities
- Oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions.
- Manage and mentor Care Coordination RNs.
- Monitor performance.
- Ensure regulatory compliance.
- Collaborate with interdisciplinary teams to optimize patient flow and effectiveness.
- Demonstrate strong leadership, strategic planning, and clinical expertise, driving continuous improvement and optimal patient care coordination for organizational goals.
