This role oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. The pharmacist will use effective patient education techniques to provide counseling to patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. They will design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens based on relevant biomedical literature, implementing recommendations effectively and utilizing policies and procedures. The pharmacist will review and interpret medication orders and prescriptions, participate in the management of medical emergencies, and actively support department medication safety and quality improvement activities. They will perform quality assurance and performance improvement of the medication use system, collaborate with the healthcare team to manage formulary adherence and the medication-use process, and assist in the acquisition of non-formulary, compassionate use, and investigational medications. Practice must be in compliance with organization’s policies and procedures, as well as local, state, and federal regulations. The pharmacist will prepare, dispense, and oversee the proper storage and handling of medications. Additionally, they will serve as a preceptor for pharmacy students, residents, and healthcare learners, and mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students. The role involves directing and checking the accuracy of pharmacy support team members' work to ensure safe and efficient workflows, service standards, and regulatory compliance. The pharmacist must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, understanding the principles of growth and development over the life span and assessing data reflective of patient status to meet age-specific needs.
