The Mail Order Clinical Pharmacist will oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. This role involves using effective patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, appropriate use, handling, and administration. The pharmacist will design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens, implementing recommendations and documenting them appropriately. They will review and interpret medication orders and prescriptions, participate in medical emergencies, and actively support medication safety and quality improvement activities. The position also includes performing quality assurance and performance improvement of the medication-use system, collaborating with the healthcare team on formulary adherence and medication-use processes, and assisting with the acquisition of non-formulary, compassionate use, and investigational medications. Practice must comply with organizational policies, procedures, and local, state, and federal regulations. The pharmacist will prepare, dispense, and oversee the proper storage and handling of medications. Additionally, they will serve as a preceptor for pharmacy students, residents, and healthcare learners, and mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students. The role involves directing and checking the accuracy of pharmacy support team members to ensure safe and efficient workflows, service standards, and regulatory compliance. The pharmacist must demonstrate knowledge and skills appropriate for the age of the patients served, understanding growth and development principles and assessing data to meet age-specific needs.
