Weekends - LVN / RN Pediatric Home Health Nurse
About The Position
Angels of Care Pediatric Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering care according to the active plan of care, administering medications and treatments, coordinating care, educating family members, and implementing preventative initiatives to ensure positive patient outcomes. Requirements
- Active RN or LPN/LVN license
- Ability to provide care in a client home setting

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care.
- Educate family members on patient clinical care to enhance positive outcomes.
- Preventative initiatives to protect quality of care for patient.
