LVN Patient Care Coordinator, Hospice

The hourly range for this position at commencement of employment is expected to be between $32 - $36; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience. Requirements
- Current California LVN or RN license preferred
- Minimum 2 years of hospice or home health patient coordination experience
- Knowledge of California hospice regulations and Medicare/Medicaid guidelines
- Current CPR certification
- Strong communication, organization, and multitasking skills
- Proficient with computers and electronic medical records
- Compassionate, patient-centered approach
- Excellent organizational and problem-solving abilities
- Ability to work independently and as part of a multidisciplinary team
- Professional demeanor with outstanding customer service skills
Responsibilities
- Coordinate daily hospice patient schedules and services
- Manage patient referrals, admissions, and intake processes
- Obtain physician orders and verify insurance benefits/authorizations
- Support interdisciplinary team meetings and coordinate patient care plans
- Monitor documentation for quality, accuracy, and regulatory compliance
- Prepare and submit required hospice quality and HIS data
- Serve as a resource for clinical staff regarding hospice patient care
- Collaborate with case managers to ensure continuity of care
- Assist with staff orientation, education, and performance improvement initiatives
- Maintain patient confidentiality and uphold hospice standards of excellence
