About The Position

The LVN Clinical Care Coordinator is responsible for supporting the coordination, monitoring, and continuity of patient care services provided by Neogen Care Corp. The position serves as a key communication link between patients, caregivers, field clinicians, physicians, referral sources, and internal clinical and administrative teams. The LVN Clinical Care Coordinator assists the clinical leadership team with monitoring patient needs, coordinating services, following up on clinical concerns, supporting timely communication, reviewing clinical documentation, and helping ensure that care is delivered according to the established plan of care, physician/allowed practitioner orders, agency policies, and applicable regulatory requirements.

Requirements

  • Current and unrestricted California Licensed Vocational Nurse (LVN) license.
  • Current CPR/BLS certification.
  • Strong knowledge of medical terminology and basic nursing principles.
  • Excellent verbal and written communication skills.
  • Strong organizational skills and ability to prioritize multiple patient and clinical needs.
  • Ability to work effectively in a fast-paced home health environment.
  • Proficiency with EMR systems and basic computer applications.
  • Ability to communicate professionally with patients, caregivers, physicians, referral sources, and clinical staff.
  • Strong problem-solving, critical-thinking, and follow-up skills.
  • Ability to recognize situations requiring escalation to an RN, Clinical Supervisor, or other appropriate clinician.
  • Must maintain all required licenses, certifications, and professional credentials.

Nice To Haves

  • Home health experience strongly preferred.
  • Previous clinical coordination, case management, utilization review, or office-based clinical experience preferred.
  • Knowledge of home health regulations, Medicare requirements, and clinical documentation standards preferred.

Responsibilities

  • Review new patient referrals and available clinical information to assist with determining appropriate services and disciplines needed.
  • Coordinate patient admissions, start-of-care visits, follow-up visits, recertifications, and other required services.
  • Assist with assigning and coordinating appropriate clinicians based on patient needs, geographic coverage, and staff availability.
  • Monitor patient schedules and help ensure timely completion of required visits.
  • Communicate with field RNs, LVNs, PTs, OTs, STs, MSWs, HHAs, and other members of the interdisciplinary team.
  • Follow up with clinicians regarding missed visits, delayed documentation, outstanding orders, and changes in patient condition.
  • Review clinical documentation for completeness, accuracy, consistency, and compliance with agency requirements.
  • Assist with monitoring physician orders, plan-of-care updates, certifications, recertifications, and other required clinical documentation.
  • Track and follow up on outstanding physician orders and required signatures.
  • Assist with coordinating communication between physicians, referral sources, hospitals, facilities, patients, and caregivers.
  • Escalate significant changes in patient condition, safety concerns, medication issues, or other clinical concerns to the appropriate RN/Clinical Supervisor.
  • Assist with coordinating hospitalizations, emergency situations, transfers, and changes in patient status.
  • Support the coordination of DME, medical supplies, laboratory services, and other patient care needs.
  • Communicate with patients and caregivers regarding visit schedules, care coordination, and non-emergency clinical concerns within the scope of LVN practice.
  • Assist with monitoring patient satisfaction and promptly address or escalate patient and caregiver concerns.
  • Participate in interdisciplinary communication and case conferences as directed.
  • Assist with tracking quality indicators, patient outcomes, missed visits, hospitalizations, and other clinical performance metrics.
  • Assist with chart audits and quality assurance activities as assigned.
  • Support compliance with Medicare, CHAP, HMO, Kaiser, and other applicable payer or regulatory requirements, as applicable to the agency.
  • Assist with onboarding, orientation, and clinical support of new field staff as assigned.
  • Provide education and guidance to patients and caregivers within the LVN's scope of practice and under appropriate clinical direction.
  • Maintain accurate and timely documentation in the agency's electronic medical record (EMR) system.
  • Maintain confidentiality and safeguard protected health information in accordance with HIPAA and company policies.
  • Participate in staff meetings, in-service training, quality improvement activities, and performance improvement initiatives.
  • Perform other clinical coordination and administrative duties as assigned by the Clinical Supervisor or Director of Patient Care Services.
