About The Position

Catholic Charities is seeking a compassionate and dedicated individual to join their family-friendly organization as a Parenting Services / Medical Coordinator. This role focuses on providing essential education and medical services to expecting parents and new parents within the Houston-Galveston area. The successful candidate will offer personalized care, adhere to program policies and licensure requirements, and ensure the well-being of minors. This position involves working closely with registered nurses, medical providers, and other team members to coordinate medical appointments, provide transportation, manage medication logs, and advocate for pregnant teens. The role also includes participation in quality improvement activities, treatment plan meetings, and ongoing training. The position is carried out in designated shelter locations, schools, and group home settings.

Requirements

  • Licensed practical or vocational nurse (LPN/LVN) or higher health-related qualification (e.g., licensed registered nurse (RN), OR Bachelor’s degree in Nursing or a related field
  • One (1) year of experience in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care setting.
  • Bilingual (English/Spanish)
  • Valid Texas Driver’s License
  • Proficient typing/keyboarding skills and computer skills including but not limited to Microsoft Office applications such as Outlook, Word, Excel, and power point.
  • Excellent communication (verbal and written) and interpersonal skills.
  • Ability to work and function well in a diverse and multidisciplinary environment.
  • Capacity to and adjust/adapt to flexible schedules, prioritize activities, responsibilities, and work within fast-paced environment.
  • Knowledge of essential medical terminology and understanding basic health attention like, vital sign measuring, blood sugar monitoring, and Weight/height measure; as well, performance hearing and vision screening.
  • Ability to adapt to changing work requirements and varying volumes of work under pressure.
  • Knowledgeable of HIPAA, state and federal regulations governing confidentiality and release of information.
  • Support and train team members under Registered Nurse supervision and guidance.
  • Demonstrate knowledge and understanding about boundaries with kids, staff and volunteers.

Responsibilities

  • Create an educational plan to facilitate classes for expecting parents and teen parents.
  • Establish a trusting and professional relationship with pregnant UCs and teen mothers with newborns, infants, and/or toddlers needing pregnancy and parenting classes.
  • Educate clients in life skills in both English and Spanish to pregnant and teen mothers.
  • Escort clients during labor and delivery at the facility/hospital and transport teen mother/child for pediatrician follow-ups.
  • Ensure the safety and security of all clients in their care.
  • Provide quality care and supervision to clients in accordance with ORR and TDFPS rules, program philosophy and procedure and client’s rights.
  • Maintain ratio and close supervision through all activities.
  • Report immediately to the supervisor any doubts, questions, medical concerns, or procedures regarding client’s health.
  • Ensure client’s information and medical files are safe and kept confidential.
  • Effectively communicate information pertaining to the health of clients according to program policies and procedures.
  • Adjust work to the plans and programs that the Registered Nurse implements.
  • Maintain accurate and updated documentation regarding biographical data, admission assessment, medical progress notes, Medication List, Medication Log, MARs, TARs, Sickness/Allergy Forms, Medical instructions/recommendations, medical summaries, Safety plans, SIRs, IRs, and others as needed.
  • Keep ongoing training to compliance with the annual training in accordance with state regulations, Office of Refugee and Resettlement, and both Catholic Charities and St. Michael’s Home for Children requirements.
  • Assist in the development, oversight, or provision of medical services for clients in care as approved by your supervisor for safety and licensure compliance.
  • Look for assistance or advice from direct supervisor if any doubts or questions before performing an assigned task.
  • Attend approved and required professional development opportunities.
  • Maintain all licenses, certificates, and/or training as required by funders, program, and/or agency.
  • Maintain client electronic records in compliance with agency policies and procedures, and all applicable legal requirements, including HIPAA.
  • Enter all applicable client information into the agency's database in a timely and accurate manner.
  • Prepare and submit all required administrative reports and documentation within given deadlines without compromising quality.
  • Participate in and be responsive to quality and compliance activities, including the agency's record review and qualitative review process, as well as incident reporting practices.
  • Perform other duties as assigned train such as participating in staff retreats, United Way tours, agency and community fairs and speaking engagements, assisting with special organizational projects/events, to promote agency programs.
