About The Position

The Crossover Nurse is a key partner in creating a strong patient experience. The position actively manages patient care by coordinating and connecting patients with specialists and their facilities, following up with patients, and guiding them through the process while modeling Crossover’s values. The work takes place in a fast-paced, rapidly changing environment and follows a philosophy centered on primary care as the patient-centered home and evidence-based medicine guidelines.

Requirements

• Licensed Vocational Nurse from an accredited school.
• Minimum of 3 years of clinical experience.
• Current BLS (Basic Life Support) certification.

Responsibilities

• Delivers patient care under the oversight of an RN or clinic supervisor.
• Collaborates with patients and the care team to facilitate a patient-centered medical home.
• Provides evidence-based nursing care at the highest community standard.
• Coordinates and connects patients with specialists and their facilities.
• Provides phlebotomy and immunization services.
• Performs other duties as assigned.
