About The Position
We are seeking a dedicated LVN/LPN for a long-term care and rehabilitation setting. This role involves providing quality nursing care to a diverse patient population, including both long-term residents and short-term rehab patients.
Requirements
- Active LPN/LVN license
- Minimum 1 year of LTC (long-term care) experience required
- BLS certification required
- Experience in skilled nursing facility or nursing home setting
- Proficiency in blood glucose monitoring/testing
- Experience with Epic charting system required
Responsibilities
- Deliver direct patient care in a skilled nursing facility
- Monitor patient conditions and perform blood glucose testing as needed
- Administer medications and treatments per physician orders
- Maintain accurate documentation using electronic medical records
- Collaborate with interdisciplinary teams to ensure quality patient outcomes
- Float between units within the facility as required
