As a Community Liaison, you’ll be at the heart of patient care and community partnership—serving as a trusted bridge between our team, referral sources, and the facilities we support. You’ll build strong, meaningful relationships while ensuring every patient’s transition is smooth, compliant, and fully supported. Requirements

  • Active LVN license in the state of California
  • Must have excellent observation, verbal and written communication skills, problem solving skills.
  • Must be able to perform prolonged or considerable walking or standing
  • Must be able to lift supplies and equipment
  • Must be able to perform considerable reaching, stooping, bending, kneeling, and/or crouching
  • Must have visual acuity and hearing.
  • Must be flexible in work hours
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Nice To Haves

  • One to two years of experience in Hospice Sales preferred.

Responsibilities

  • Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
  • Continuously conduct audit and review of charts to ensure compliance.
  • Establish and monitor community, customer, and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
  • Receive hospice referrals.
  • Conference with Clinical Supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
  • Follow intake referral processes.
  • Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services
  • Promptly respond to customer and referral source requests and concerns.
  • Complete consent forms as assigned and indicated.
  • Submit associated documentation in accordance with administrative processes.
  • Provide patient/family education related to hospice philosophy and care.
  • Comply with accepted professional standards of practice and principles.
