About The Position

Continuum Hospice is seeking a dynamic and dedicated nurse to join our team as a LVN Hospice Liaison. If you have a background as a hospice clinician, a strong interest in expanding your career into marketing and sales, and are passionate about our hospice mission, we invite you to consider this exciting opportunity. As a LVN Hospice Liaison, you will support and partner with current Liaisons to identify new referral sources and maintain ongoing relationships to increase program census and preference for hospice services. LVN Hospice Liaison use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our LVN Hospice Liaison's are hospice experts and serve as a resource to our Sales team and referral sources to ensure extraordinary patient and customer service.

Requirements

  • Current unencumbered license as a licensed vocational nurse (LVN) or licensed practical nurse (LPN).
  • Minimum 1 year of experience working directly in hospice required.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Nice To Haves

  • 2-3 years' experience preferred.
  • Prior customer service or sales experience strongly preferred.

Responsibilities

  • Collaborate with our Liaisons to gather crucial patient clinical information, provide admissions support, and actively participate in community events.
  • Join our Liaisons in brand awareness campaigns to make our program the preferred hospice provider in the service area, fulfilling our mission and achieving agency growth and census management goals.
  • Conduct initial meetings with patients and their families, introducing them to the hospice service and our compassionate philosophy, and obtain consent for their care.
  • Ensure compliance with regulatory standards through meticulous record-keeping and prompt and accurate submission of marketing reports.
  • Identify key accounts and help shape our strategy by communicating strengths and weaknesses to our Liaisons and Regional Sales Director.
