About The Position

The LVN, Case Manager Liaison coordinates efficient and effective transitions for patients from the hospital setting to the home environment while maximizing patient outcomes. The role requires knowledge and understanding of pediatric home care. The position is with Home Health in Fort Worth, in the Administration department, and works the first shift for 40 standard weekly hours.

Requirements

• Formal education from an accredited college or university in nursing.
• Education and job training as a Licensed Vocational Nurse.
• Current State of Texas LVN licensure or eligibility for licensure.
• At least 3 years of pediatric experience.
• Basic life support (BLS) certification or scheduled attendance of a CPR certification course.
• Knowledge and understanding of pediatric home care.
• Maintain a primary residence within Texas and all required licenses and credentials throughout employment.

Nice To Haves

• 2 years of home health and/or discharge facilitation/case management experience.
