About The Position

The LVN, Case Manager Liaison coordinates efficient and effective transitions for patients from the hospital setting to the home environment while maximizing patient outcomes. The role requires knowledge and understanding of pediatric home care. The position is with Cook Children’s Home Health in Fort Worth and is scheduled for a first shift, with 40 standard weekly hours.

Requirements

• Formal education from an accredited college or university in nursing.
• Education and job training as a Licensed Vocational Nurse.
• Current State of Texas LVN licensure, or eligibility for licensure.
• At least 3 years of pediatric experience.
• Knowledge and understanding of pediatric home care.
• Basic life support (BLS) certification or scheduled attendance of a CPR certification course.
• Maintain a primary residence in Texas throughout employment.
• Maintain all licenses and credentials required for the position throughout employment.

Nice To Haves

• 2 years of home health and/or discharge facilitation/case management experience preferred.
