About The Position

Utilizes the nursing process to perform skilled nursing functions in accordance with the Nurse Practice Act, Federal and State guidelines. Provides direct skilled nursing care to patients in their home under the supervision of a Registered Nurse, supervised by the Director of Patient Care Services and reports status. Provides services in accordance with Agency policies, standards and practices. Responsible in following plan of care, provides treatment and works with the team to meet positive results for patient care.

Requirements

Currently licensed by the California Board of Nursing to practice as a Licensed Vocational Nurse.
Minimum of one year in the acute care setting.
With knowledge of compliance to accepted professional standards and practice.
Must be able to maintain strict HIPAA confidentiality with regard to protected and sensitive information.
Must be able to solve problems independently and confidently by applying analytical and logical thinking.
Skilled in assessing and reporting in an accurate, concise and professional manner.
With good oral and written communication skills.

Nice To Haves

Two years experience in Home Health preferred.

Responsibilities

Coordinates with home health services to the registered nurse/DPCS/Designee by informing the needs for the other disciplines - CHHA, MSW, PT, OT, ST, Registered Dietician.
Gives skilled nursing care in the home as ordered on the Medical Plan of Treatment and Nursing Care Plan.
Performs aseptic techniques as required.
Performs specific treatments and administers medications as ordered.
Educated patients and significant others regarding the patient's illness, procedures, treatments, including self-care, prevention, and health maintenance.
Assists patient and/or family in keeping home and environment safe.
Keeps patients supplied with necessary materials and sees that charges are submitting for billing.
Prepares meals for patient appropriately.
Records information accurately, legibly, and concisely following the current Medical Plan of Treatment.
Documents all changes in patient's physical or psychosocial condition.
Attends staff meetings, patient conferences, and takes an active part in planning patient care as needed.
Documents and notifies staff on changes in patient's medications, treatments, physical and psychosocial condition.
Provides discharge instructions to patient and family.
Keeps accurate visit records of time.
Attends and participates in continuing education programs for personal and professional growth and development.
Adheres to all Company policies and procedures.
Demonstrates flexible and efficient time management techniques.
Prioritizes workload so as to meet productivity and standards and applicable deadlines.
Maintains a positive and respectful attitude and demeanor.
Maintains strict HIPAA confidentiality with regard to protected and sensitive information.
Submits accurate and timely documentation in accordance to the plan of care.
Uses medical equipment and supplies efficiently and effectively.
Performs other duties or tasks as required as delegated by the RN Supervisor.
