About The Position

This role involves building positive relationships with residents in a safe, structured, and person-centered living environment. The coordinator will assist the Residential Manager with daily services, resident routines, and activity programming. They will act as a resource for team members regarding activity and engagement strategies, ensuring consistency across shifts. The position requires developing, planning, and leading structured daily activities, including individual, group, and culturally relevant programming. Collaboration with LTC staff and the Community Health Dietician for weekly meal planning, grocery shopping, and meal preparation is also a key responsibility, aimed at supporting resident participation, life skills development, and healthy routines. The role supports daily wellness and resident engagement through recreation, structured activities, and individualized interactions. Assistance with activities of daily living (ADLs), including personal care, dressing, grooming, and mobility, may be required. The coordinator will also support the development of independent living skills through daily routines like cooking and light housekeeping. Observing resident wellness and communicating changes or concerns to appropriate team members is essential. Documentation of services provided, resident participation, and observations in the electronic health record (EHR) is required.

Requirements

  • Associate’s degree in human services, healthcare, or related required.
  • Must be 21 years or older.
  • Must have a valid driver’s license and meet eligibility requirements for Tribal vehicle use.
  • Must successfully pass all applicable background checks and drug screens.
  • Proof of immunizations is required and immunizations/testing provided upon hire.

Nice To Haves

  • Bachelor’s degree preferred.
  • Experience developing, coordinating, and/or implementing culturally relevant programming that reflects Forest County Potawatomi Community values and traditions preferred.
  • Nursing Assistant (CNA) experience preferred.
  • Experience in in-home care, residential care, or similar setting preferred.
  • Experience supporting or leading activities for individuals with varying support needs preferred.
  • Medication administration experience preferred; ability to complete competency evaluation prior to independent medication passing required.

  • Documents services provided, resident participation, and observations in the electronic health record (EHR) in accordance with program requirements.
