About The Position

Builds positive, respectful relationships with residents while supporting a safe, structured, and person-centered living environment. Assists residents with activities of daily living (ADLs), including personal care, dressing, grooming, and mobility. Observes resident wellness and communicates changes or concerns to appropriate team members. Collaborates with the House Manager, Lead CHW/Activities Coordinator, and Community Health staff, including RN case managers, to support coordinated care. Supports medication routines in accordance with program guidelines. Supports daily wellness and engagement through recreation, activities, and individualized interactions. Supports development of independent living skills through household routines, including cooking and light housekeeping. Assists with community outings, appointments, and activities that promote inclusion and participation. Documents services provided, resident participation, and observations in the electronic health record (EHR) in accordance with program requirements.

Requirements

  • High School Diploma or GED required.
  • Ability to provide direct resident support, including activities of daily living (ADLs).
  • Must be 18 years or older and have a valid driver’s license and meet eligibility requirements for Tribal vehicle use.
  • Must successfully pass all applicable background checks and drug screens.

Nice To Haves

  • Nursing Assistant (CNA) experience preferred.
  • Experience in in-home care, residential care, or similar setting preferred.
  • Experience supporting or leading activities for individuals with varying support needs preferred.
  • Medication administration experience preferred; ability to complete competency evaluation prior to independent medication passing required.
