About The Position

The LPN assures the quality and continuity of resident care and supervises the performance of nursing care team members on a designated Neighborhood and shift. The role involves coordinating resident care, monitoring residents’ conditions, communicating with families and physicians, and working collaboratively with nursing staff and other departments. This is a part-time night-shift position, scheduled from 10:45 p.m. to 7:15 a.m., with every other weekend and holiday worked (four days per pay period). A $6,000 sign-on bonus is available to those who qualify.

Requirements

Active, unencumbered Pennsylvania LPN license.
High school diploma or GED equivalent.
Proficiency with Microsoft Office Suite, POC, and electronic health record systems.
Strong interpersonal and communication skills, including effective communication with residents, families, interdisciplinary teams, and external partners in person and by phone.
Effective verbal and written communication across diverse audiences.
Strong organization and attention to detail, with the ability to maintain accuracy in a fast-paced environment with frequent interruptions.

One year of clinical experience; preferred but not required.

Responsibilities

Help provide a safe and pleasant environment and maintain high morale among residents and nursing personnel.
Initiate disciplinary procedures for staff as needed.
Establish and maintain effective interdepartmental working relationships and promote guest relations.
Arrange resident appointments and transportation to other departments or outside appointments.
Observe and evaluate residents’ symptoms and conditions; ensure families and physicians are updated and physician orders are followed.
Plan, implement, and complete residents’ plans of care, revising them continuously after admission.
Supervise the performance of nursing care team members on the designated Neighborhood and shift.
