Requirements
- Active Oklahoma LPN license
- Valid driver's license and a clean driving record

- Home health experience preferred, but if you have solid clinical skills and a heart for patients, we'll teach you the rest
Responsibilities
- Provide skilled nursing visits under the direction of your RN partner and the plan of care
- Perform wound care, medication management, vital signs, and disease management
- Teach patients and families how to manage their health at home
- Report changes in condition to your RN partner and the care team
- Document accurately and on time
