Requirements

• Active Ohio LPN license required
• G-tube experience required
• Compassionate and patient-centered approach
• Strong clinical skills and attention to detail
• Dependable and professional work ethic
• Comfortable working independently in a home setting
• Reliable transportation
• Ability to pass required background and drug screening

Responsibilities

• Provide individualized, patient-centered nursing care in the home
• Provide safe and attentive G-tube care
• Administer medications and treatments as ordered
• Monitor the client's condition and communicate changes to the care team
• Follow the individualized plan of care
• Maintain accurate clinical documentation
• Create a calm, respectful, and supportive environment
• Build a positive relationship with the client and family
