About The Position

The LPN will provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring. This role involves observing, assessing, and reporting changes in resident condition to physicians and the nursing team. The LPN will also assist in the development and modification of resident care plans, document resident care accurately in electronic health records, and collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care. Participation in facility surveys and quality improvement initiatives, as well as maintaining a safe and therapeutic environment for residents, are key aspects of this position.

Requirements

Current, unencumbered LPN license.
Ability to make independent decisions and exercise sound judgment.
Strong communication and interpersonal skills, with the ability to interact tactfully with residents, families, and staff.
Knowledge of nursing and medical practices, procedures, and long-term care regulations.
Ability to work effectively in a team environment and supervise other personnel as needed.
Patience, compassion, and a positive attitude.
Ability to lift, push, pull, and transport residents with assistance as needed.
