The LPN Team Lead provides nursing care to CCHS patients, assisting physicians and nurse practitioners with various aspects of patient care and performing patient testing as ordered. The LPN helps ensure smooth patient flow through the clinic and also assists with clerical responsibilities. As Team Lead, this role also serves as a working lead for other LPNs on the care team — providing daily work direction, training, and competency validation. (Formal supervisory authority, including discipline, rests with the LPN/RN Clinical Supervisor.) As a faith-based Federally Qualified Health Center, CCHS expects every team member to embody our core values in daily practice. These values are explicit, measurable performance expectations — how the work is done matters as much as what is done. Intentional Love — Extending compassionate, whole-person care to every patient, family member, and colleague. In this role it means bringing that same compassion to every draw, exam prep, and phone call, so patients feel cared for at every step of their visit. Devoted Service — Approaching every responsibility with humility, reliability, and a servant's heart. In this role it means being dependable for your care team and the LPNs you help lead — from morning huddles to end-of-day cleanup. Humble Excellence — Pursuing the highest standard of clinical and professional quality while remaining teachable and Christ-centered. In this role it means holding a high bar for clinical accuracy and safety while mentoring newer LPNs with patience and grace. Employees in this role are expected to model these values in patient interactions, team collaboration, and community engagement, in support of CCHS's healing ministry.
