About The Position
Hammonton Center is hiring an LPN Supervisor in Hammonton, New Jersey. The role oversees facility operations during weekend night shifts, including resident care coordination, staff workflow, medication assistance, incident reporting, admissions, and shift communications. Hammonton Center for Rehabilitation and Healthcare is a 240-bed rehabilitation and skilled nursing facility in Hammonton, New Jersey. Requirements
- Must be able to work as a team member.
- Valid state LPN license.
- Must be in good standing with the State Registry.
Responsibilities
- Conduct facility rounds.
- Monitor aides’ assignments to ensure proper workflow.
- Supervise medication assistance times to ensure assistance and refusals are properly recorded, and initiate necessary follow-up.
- Intervene in resident conflicts, outbursts, and behavioral issues.
- Facilitate communication so medical and mental health situations are addressed promptly.
- Ensure all aspects of facility incidents and accidents are completed, and provide a summary to the EHP Coordinator or Administrator.
- Attend to late admissions and readmissions, including paperwork and medication changes.
- Communicate pertinent shift activity through a 24-hour report for review by the Enriched Housing Program Coordinator or Administrator.
- Facilitate communication about environmental situations, contact the EHP Coordinator or Administrator when necessary, and record details for their review.
