About The Position

JOB SUMMARY: Assists with coordinating treatment to provide comprehensive wound assessments. Follow plan of care. Provide scheduled wound treatments as ordered by physician. Assists with patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Assists with performance improvement plan of the organization.

Requirements

Graduate of state approved school of practical nursing and has passed the state board examination.
Current state License Practical Nurse Licensure.
Minimum of two (2) years’ experience in long term care nursing.
Current BCLS Certification
Ability to communicate in English, both verbally and in writing.
Basic Computer Skills

Responsibilities

Performs all job responsibilities in alignment with the mission and vision of the organization.
Performs other duties as required and completes all job functions as per departmental policies and procedures.
Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs).
Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
Wears protective clothing and equipment as appropriate.
Ability to perform wound assessment, reassessment and management of all patients and per policy. This includes geriatric and the general population.
Knowledgeable of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.
Utilizes principles of current wound management theory when providing care to patient with acute and chronic wounds.
Demonstrates thorough knowledge of staging system of wounds.
Follows skin care programs to prevent skin breakdown of patient.
Ability to follow plan of care.
Ability to adequately assess and reassess pain.
Demonstrates ability to perform treatments and provide services to level licensure.
Demonstrates ability to assist with a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
Knowledgeable of medications and the correct administration based on age of the patient and clinical condition.
Follows the five (5) medication rights and reduces the potential for medication errors.
Identifies and addresses psychosocial needs of patients and families; communicates with social service/discharge planner as appropriate.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Demonstrates ability to assist with in-inservices on wound prevention, wound care, ostomy care and incontinence for the nursing staff.
Communicates appropriately and clearly to nurse managers, nursing staff and physicians.
Consults other healthcare professional/departments as appropriate to provide interdisciplinary approach to the patient’ needs.
Documentation meets current standards and practices.
Stays current with changes, new advances/treatment in wound care.
Follows Code of Conduct policy.
Adheres to dress code; appearance is neat and clean.
Completes annual educational requirements.
Maintains regulatory requirements.
Maintains patient confidentiality at all times.
Reports to work on time and as scheduled; completes work within designated time.
Wears identification when on duty; uses computerized time clock system correctly.
Completes in-services and returns in a timely fashion.
Attends annual review and/or skills fair and department in-services, as scheduled.
Attempts to end conversations and other interactions in a positive manner; leaves others with a good impression of the Hospital Authority of Miller County and its employees.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission statement of the organization.
