About The Position

This position involves conducting daily nursing functions in accordance with current rules, regulations, and guidelines governing long-term care facilities and assisted living communities. The role requires ensuring compliance with facility policies and procedures, managing resident admissions, transfers, and discharges, and participating in resident and family orientation. Key responsibilities include conducting rounds, communicating with healthcare providers, developing and updating care plans, administering medications, and supervising nursing unit personnel. The role also emphasizes maintaining a safe and sanitary environment, managing supplies, and performing other assigned duties. The position requires independent decision-making, effective communication, and the ability to work with diverse resident populations. It necessitates adherence to all state and federal requirements for LPNs and may prefer one year of experience in a home care, assisted living, or long-term care setting.

Requirements

  • Must meet all applicable state and federal requirements for this position.
  • Must possess a current, unencumbered, active license to practice as an LPN in the applicable state, in good standing throughout employment with no disciplinary action in effect against professional license.
  • Be able to make independent decisions and follow instructions.
  • Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public.
  • Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.
  • Communicate effectively in a manner that is sufficient for effective communication with supervisors, team members, prospects, residents, and families.
  • General computer literacy.
  • Be able to lift up to 50 pounds at times.
  • Be able to push and pull up to 50 pounds at times.

Responsibilities

  • Conduct daily nursing functions in accordance with current rules, regulations, and guidelines.
  • Ensure nursing personnel comply with written policies and procedures, including the safety program.
  • Admit, transfer, and discharge residents with appropriate documentation.
  • Participate in the orientation of new residents and their families.
  • Make rounds with healthcare providers, Hospice staff, and Home Health staff as needed.
  • Utilize nursing personnel qualifications according to resident care plans, service plans, and healthcare provider orders.
  • Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
  • Meet and resolve issues with residents and/or family members.
  • Report significant issues to the supervisor.
  • Participate in facility surveys as requested by management.
  • May be responsible for clinical programs such as dementia, wound care, quality assurance, infection control, and admissions.
  • Complete initial assessments and assist or develop interim care plans/service plans.
  • Update, review, and transcribe care plans identifying goals, problems, approaches, and revisions.
  • Assist with service plans as directed by the Director of Nursing.
  • Communicate with healthcare providers to review treatment plans, complete orders, and progress notes.
  • Review resident medical records for specific treatments, medication orders, diets, labs, etc., for implementation and follow-up.
  • Requisition and arrange for diagnostic and therapeutic services as ordered by the healthcare provider.
  • Communicate department policies and procedures to personnel, residents, and visitors.
  • Provide written (24-hour report) and oral reports/recommendations concerning resident condition changes and shift activities to nursing management.
  • Complete and submit accident/incident reports involving residents to the Director of Nursing.
  • Complete assessments and all other medical record documentation as required.
  • Provide resident and family information for care conferences or other meetings.
  • Report discrepancies in physician's orders, diet changes, charting errors, etc., to the supervisor.
  • Complete clinically accurate documentation according to policy.
  • Review resident medical documentation and progress notes from the interdisciplinary team to determine if care plans/service plans are being followed.
  • Report concerns to the supervisor.
  • Inform nursing personnel of new admissions and resident care needs.
  • Ensure rooms are ready for new admissions.
  • Greet newly admitted residents upon admission.
  • Notify the resident’s health care practitioner and family of any change in resident condition.
  • Assist in arranging transportation for residents and other third-party services.
  • Prepare and administer medications ordered by the healthcare provider.
  • Communicate with the pharmacy to ensure resident medications are available.
  • Ensure an adequate supply of floor stock medications, supplies, and equipment is on hand.
  • Report supply needs to the supervisor.
  • Review medication administration records for completeness and accuracy.
  • Dispose of drugs and narcotics as required and in accordance with established procedures.
  • Supervise the nursing unit to ensure personnel perform work assignments according to acceptable nursing standards.
  • Work with the Director of Nursing to resolve staff performance issues.
  • Report complaints and grievances to management and work with the Director of Nursing for a plan of action.
  • Assure and make available protective equipment for staff.
  • Maintain nursing service work areas, resident rooms, and treatment areas in a clean and sanitary manner.
  • Recommend equipment and supply needs to the Director of Nursing.
  • Ensure a stock level of medications, medical supplies, and equipment is maintained.
  • Perform other job-related duties as assigned.
