About The Position

Consumer Direct Care Network is seeking a compassionate and skilled Home Care Nurse to provide personalized skilled nursing care to members in their homes. The role involves assessing members’ health, developing and implementing individualized care plans, administering prescribed medications and treatments, documenting care, educating members and families, and coordinating with other healthcare professionals. This is a part-time position in Missoula, Montana, with expected hours of 10–40 per week and hourly pay of $28–$38.

Requirements

• Valid nursing license in Montana.
• Excellent communication, documentation, and interpersonal skills.
• Ability to work independently and prioritize tasks effectively.
• Current driver’s license and auto insurance.
• Current BLS certification.
• Strong knowledge of human anatomy and physiology.
• Competence in medication administration.
• Adherence to professional and industry standards of practice and care.

Responsibilities

• Conduct comprehensive assessments of members’ health status.
• Develop and implement individualized care plans.
• Observe, identify, and document changes in clients’ physical, emotional, mental, and social conditions; reactions to medications; and environmental safety factors.
• Administer medications and treatments as prescribed.
• Maintain accurate and timely records and protect the confidentiality of medical records.
• Educate members and their families on healthcare procedures and disease management.
• Collaborate with other healthcare professionals to support coordinated and effective care.
