About The Position

The Licensed Practical Nurse (LPN) provides compassionate, person-centered nursing care and health support to individuals with Intellectual and Developmental Disabilities (IDD). This role ensures the health, safety, and well-being of individuals by delivering direct clinical care, administering medications, and supporting each person’s individualized care plan.

Requirements

  • Graduated from an accredited school of nursing.
  • Current, valid Licensed Practical Nurse (LPN) license in Georgia
  • Minimum of 2 years experience serving individuals with developmental disabilities.
  • CPR and First Aid certification (or ability to obtain upon hire)

Responsibilities

  • Provide direct nursing care, including monitoring vital signs, assessing health status, and responding to medical concerns.
  • Administer medications and treatments according to physician orders and in compliance with regulatory standards.
  • Maintain accurate documentation of all nursing care, treatments, and medication administration in medical records.
  • Assist in the development and implementation of person-centered care plans.
  • Review health data tracking logs.
  • Assists with interpretation of physician, nurse practitioner or physician assistant orders and reviews summary report after office visits.
  • Liason(if needed) between individual and physician, nurse practitioner or physician assistant.
  • Responsible to verify accuracy of pharmacy orders, transcribing prescription orders and documenting into the MAR.
  • Provide first aid and emergency care when needed; coordinate care with physicians and other healthcare professionals.
  • Support individuals with IDD in achieving the highest possible level of independence and quality of life.
  • Educate individuals, families, and staff on health-related topics, including hygiene, nutrition, and preventive care.
  • Advocate for the rights and dignity of each individual, ensuring all care is respectful and culturally sensitive.
