This is a Private Duty LPN role in Warsaw, IN, with a 3-day per week schedule, focusing on one patient for consistent, full-time hours. It offers a different pace than facility work, with 1:1 care in the home, no heavy assignments, and no constant interruptions. The role provides focused care for one patient with a set schedule every week, lower stress than facility work, strong clinical support, and more control over your day. Requirements

• Active LPN license
• 1+ year of nursing experience
• Strong observation and communication skills
• Reliable transportation
• Ability to assist with patient mobility, up to 50 pounds

Responsibilities

• Provide 1:1 nursing care in the home
• Follow the plan of care and physician orders
• Monitor patient condition and report changes
• Document care accurately and on time
• Communicate with RN, physician, and care team
